The Youth Homelessness Demonstration Project, (YHDP), is a two-year renewable demonstration grant awarded by the Department of Housing and Urban Development (HUD) to select communities nation-wide. Broadly, YHDP funds are used to specifically support youth and young adults through the age of 24.
In October of 2022, HUD announced that the Sacramento Continuum of Care (CoC) received $6.5 million to build a region-wide plan and fund innovative programs designed to end youth homelessness. From the start, our community sought to include and center young people with lived experience and expertise. The Sacramento Youth Action Board (YAB), a youth-led committee of the Sacramento Continuum of Care (CoC) Board, facilitated a series of community listening sessions to collaborate on a plan and identify gaps in youth homelessness programming. YAB members, adult advocates, education partners, and TAY service providers joined forces and held a series of Planning Team meetings from January to April 2023. These meetings ultimately resulted in the development of the YHDP Coordinated Community Plan (CCP).
Sacramento’s Coordinated Community Plan (CCP) to prevent and end youth homelessness is the result of more than six months of dedicated effort from our Youth Action Board (YAB) and Community Stakeholders. Their unwavering commitment and collaboration have led to the creation of this comprehensive plan, which will serve as our beacon of guidance in the years ahead.
The CCP serves a vital purpose in keeping our community on track and attuned to the urgent needs of unhoused youth and young adults. It is designed to address their most critical needs effectively and efficiently. As a living document, the CCP remains adaptable and flexible, capable of being updated and refined as the circumstances and demands of our community evolve.
Together, with the CCP as our foundation, we can forge a brighter future, where every youth and young adult in Sacramento has access to safe and stable housing, comprehensive support services, and the opportunities they need to not just survive, but thrive.
