HMIS Access Request Process

Homeless Management Information System (HMIS) is a secured system for Sacramento and Yolo Counties.  All users must have passed a background check, complete HMIS training, and pass their HMIS quiz before being provided access to the system. To request, upgrade, deactivate, or transfer user access please follow these steps:

  1. Supervisors: Submit the  HMIS Access Request Form.
  2. Supervisors & Users: E-sign your assigned sections on the Staff Verification for HMIS Access document you’ll automatically receive in your email inbox.
  3. Users: If you are a new HMIS user, complete the training & quiz sent to your email before the 2 weeks deadline.
  4. Users: Pass your HMIS quiz and receive login credentials from HMIS staff.
  5. Users: Please login within 72 hours and complete the HMIS End User Agreement on your first sign-in.

HMIS Training Documents and Videos

All HMIS users must review the following documents and watch the training videos to complete your HMIS training.  Once you have completed your training, please refer to the email (see step #5 above) with directions on how to take your quiz.  The quiz is open book, so you can use any of these materials to find answers.  If you have any questions about your training, please email the HMIS Department or visit us during our open office hours.
