Case Conferencing is an action-oriented program that improves coordination among homelessness service providers to swiftly transition individuals into housing. The program operates in three-month cycles with weekly meetings (12 meetings per cycle). Each cycle focuses on a different group of clients based on provider submissions. These meetings leverage the groups’ collective expertise to address service needs effectively.
Typical focus areas include: income support (supplemental security income applications, CalWorks, General Assistance, and employment resources), healthcare (CalAIM Enhanced Care Management, mental health programs, mobile medicine vans, primary care), and housing (Coordinated Access documentation, low-income or voucher housing, shared housing, and programs for poor credit and evictions).

Every Friday, 11:00 a.m.–12:30 p.m.
This meeting welcomes all providers working with individuals experiencing homelessness, regardless of client status. Providers can discuss challenging cases or resource needs, network with peers, and learn about new community resources, enhancing their ability to support those in need.
Every Monday, 1:00 p.m.–2:30 p.m.
This Case Conferencing group focuses on individuals living in shelters. The group operates with a pre-set client list, updated every three months. Providers interested in submitting a client or inquiring about the current list are encouraged to participate.
Every Tuesday, 10:00–11:30 a.m.
This Case Conferencing group focuses on both sheltered and unsheltered families experiencing homelessness. The group operates with a pre-set client list, updated every three months. Providers interested in submitting a family or inquiring about the current list are encouraged to participate.
Every Thursday, 10:00–11:30 a.m.
This Case Conferencing group focuses on single adults and couples experiencing homelessness. The group operates with a pre-set client list, updated every three months. Providers interested in submitting a client or inquiring about the current list are encouraged to participate.
Every Wednesday, 1:00–2:30 p.m.
This Case Conferencing group focuses on veterans with VASH (Veterans Affairs Supportive Housing) vouchers. The group operates with a pre-set client list, updated every three months. Providers interested in submitting a client or inquiring about the current list are encouraged to participate.
Every Thursday, 3:00 p.m.–4:00 p.m.
This Case Conferencing group focuses on veterans experiencing homelessness. The group has an open client list, allowing providers to present their clients for discussion at any call. Representatives from all veteran-specific providers, including the VA (Veterans Affairs), attend to facilitate quick connections and support for clients.
First and Third Thursdays, 1:00–2:30 p.m.
This Case Conferencing group focuses on youth experiencing homelessness (ages 18-24). Run by the youth provider Waking the Village, it offers a platform to discuss and support young clients. To discuss a client, please contact shelly@wakingthevillage.org.
